Go to www.olympus.co.uk/e510cashback and complete and submit the promotion claim form by inserting your name, address, email address, contact telephone number, date of birth and date and place of purchase, serial number by 15th January 2008, and (b) print off the promotion claim form, complete this and post together with the warranty card showing the product serial number with your original proof of your qualifying purchase (meaning a receipt or other standard proof of purchase in the event that there is no receipt proving the purchase) to Olympus E-510 Cash Back Offer, Spare Pair Marketing, 3 Howden Road, South Norwood, London, SE25 4AS so that it is received by 15th January 2008.
